    We are seeking an experienced and enthusiastic Mental Health Practitioner, to join our friendly and professional team, currently based at new premises in Leigh Park Community Centre, due to the growth of our team, with a hub at Basepoint in Gosport. Our Crisis Service covers a 24/7 week service covering Havant, Waterlooville, Bordon, Petersfield, Fareham and Gosport therefore a car driver is essential. This role will be focused on working within our Crisis and Assessment Team, providing a 4 hour response to service users who present in a self defined crisis to complete holistic assessments and make a comprehensive decision on treatment required such as Intensive Home Treatment Team or admission to Hospital.

    We also provide clinical support to the developing Safehaven Service, also based in Leigh Park 7 days a week, out of hours, alongside MIND and Inclusion. The service will provide an out of hour’s responsive assessment and treatment facility for service users presenting in a self defined crisis.

    As a member of the Crisis Urgent Assessment and Home Treatment Team, you will ensure that you offer responsive, evidence based and recovery focused care to the service users. You will work as part of a multi-disciplinary team, which supports individuals at home who are experiencing an acute mental health crisis and who require intensive input in order to remain in the community, averting hospital admission where possible. Whilst also supporting individuals along their journey through the Acute Care Pathway from the hospital environment to home.  Our team is made up of Nurses, Social Workers, Occupational Therapists, Health Care Support Workers, Psychology support, medical and admin staff.

    Training and partnership working is also available to enhance your learning through brief psychological interventions. As this role requires you have access to a vehicle Southern Health also offer a lease car scheme for community workers (T&C apply)

    A DBS check is required by the Employer for this post. The Employer will undertake an initial check at the appropriate level on appointment. As a condition of employment you are required to register with the online DBS update service and maintain this registration throughout employment.

    Frontline Healthcare workers (who have face-to-face contact with service users) in England may be required to ensure they are fully vaccinated against COVID-19, from 1 April, subject to Government consultation to revoke the regulations.

    To work as a frontline healthcare worker from 1 April 2022 currently it will be a condition of employment to be fully vaccinated. This requirement will also apply to those in ancillary support roles such as porters or receptionists who may have social contact with patients but are not directly involved in their care.

    To be fully vaccinated you must receive both doses of a Covid-19 vaccination, therefore the latest date for a first vaccination for frontline healthcare workers is 3 February 2022.

    Successful applicants will be required (unless exempt*) will be asked to provide evidence of their covid vaccination status (NHS COVID Pass) or evidence that they have commenced their journey to becoming fully vaccinated by 1st April 2022 as part the pre-employment check process i.e., booked appointment or first vaccination received.

    If the required evidence cannot be provided then any offer of employment may be withdrawn.
