Acute Frailty Pathway & Geriatric Medicine
A formal, system-wide Acute Frailty Pathway is set up to provide short term assessment and stabilisation of frail older people attending NUH, & providing a better experience & improved quality of care.
The team provides early implementation of Comprehensive Geriatric Assessment (CGA); proactively managing admissions across the designated NUH assessment ward for older people and provide specialist review of frail older people from the Emergency Department (ED).
The post offers an outstanding opportunity to develop specialist knowledge and skills for the management of frail and geriatric patients on a large and busy admissions unit as part of the friendly, progressive and dynamic multidisciplinary team.
You will be supported to advance your specialist knowledge and effectively utilise your non-medical prescribing skills.
Participating in consultant-led ward rounds to promote & support medicines optimisation & antimicrobial stewardship
Contributing to CGA
Facilitating smooth transfer of medication & related issues across the interface at admission, discharge & transfer
Ensuring patients receive the best pharmaceutical care and managing potentially inappropriate polypharmacy
Involvement in education & training of a variety of healthcare professionals including diploma & pre-registration pharmacists, undergraduates and trainee ANPs
The successful candidate must be a highly motivated team player with good interpersonal and leadership skills, and aspirations to develop as an independent practitioner
The ideal candidate will be an experienced prescriber having worked in a variety of medical specialties, and have experience of delivering a service to a specialty will have completed a clinical diploma and have experience in acute hospital pharmacy setting
